Pennsylvania's Home and Community-Based Services, Explained
Pennsylvania delivers much of its long-term care outside institutions through a set of Medicaid waiver programs and state-funded services. Here is how that system is organized and who decides who is eligible.

The shape of the system
The Pennsylvania Department of Human Services administers Medicaid home and community-based waivers. For people with intellectual disabilities, the main programs are the Consolidated waiver, the Community Living waiver, and the Person/Family Directed Support waiver. For adults with autism, there is the Adult Autism Waiver.
For adults 60 and older, the Department of Aging oversees the OPTIONS program and a statewide network of Area Agencies on Aging, alongside other in-home services funded through the Older Americans Act.
Who decides eligibility
Eligibility determinations rest with public agencies, not with service providers. For the intellectual disability waivers, the county mental health and intellectual disability office handles intake and assessment. The Adult Autism Waiver has its own statewide enrollment process. The Area Agencies on Aging determine eligibility for OPTIONS.
Some waivers maintain waiting lists, and enrollment can take time. Provider organizations deliver services once a person is found eligible and has an approved plan.
How families use the information
Once a waiver is approved, a supports coordinator or service coordinator helps build a plan and select providers. Families commonly compare providers on staffing consistency, communication, and how backup coverage works.
Private-pay care is a separate route that does not depend on waiver eligibility. Many households begin with private arrangements while an application is pending, then adjust once a determination is made.
