Clinical Director (RN or LPN)
Responsibilities: Manages nursing services for elderly clients, including chronic disease management, medication coordination, and supervision of nursing staff.
Qualifications: RN or LPN with geriatric care experience.
Case Manager / Care Coordinator
Responsibilities: Creates personalized care plans for elderly clients, connecting them with services like physical therapy, social services, and palliative care.
Qualifications: Degree in Social Work (BSW/MSW) or Nursing, with experience in geriatric care coordination.
Direct Support Professionals (DSPs)
Responsibilities: Assists elderly clients with daily activities such as grooming, dressing, meal preparation, medication reminders, and companionship.
Qualifications: Training in elderly care, personal care, and dementia support.
Occupational Therapist (OT)
Responsibilities: Helps elderly clients improve or maintain their ability to perform everyday tasks such as bathing, dressing, and mobility.
Qualifications: Licensed OT with experience in geriatric care.
Physical Therapist (PT)
Responsibilities: Develops plans to maintain or improve mobility, strength, and balance in elderly clients, preventing falls and promoting physical independence.
Qualifications: Licensed Physical Therapist (PT) with expertise in geriatrics.
Speech-Language Pathologist (SLP)
Responsibilities: Provides speech therapy for elderly clients, especially those with speech, swallowing, or cognitive impairments due to aging.
Qualifications: Licensed SLP with experience in elderly care.
